As a courtesy, we will file your insurance (except Tricare). All patients will be responsible for paying for all services not covered by your policy, deductible amounts, and co-pays. If we are not a contracted provider with your insurance, you will be responsible for paying charges in full on the date of service. It is the patient’s responsibility to confirm with the insurance company if we are a contracted provider. Payment plans may be worked out for patients experiencing financial difficulties.
All patients are requested to bring their insurance cards to each appointment. Please remember that your insurance coverage is a contract between you and your insurance company. Responsibility for payment of fees is the direct obligation of each patient.
