Currently, we participate in several managed care programs. These contracts are periodically reviewed and changed; therefore, prior to your visit, please check with your insurance company to see if we are currently participating with your plan. If required by your plan, please be sure your primary care doctor has forwarded your authorization paperwork by contacting your primary care doctor prior to your appointment to be sure your visit has been approved by your primary doctor.
